We brought partners together, clarified care pathways, and built shared systems that strengthen coordination and create a more connected, person-centered experience for Erie County Mental Health Department and its local service providers.

We helped the Community Health Worker Association of Rochester (CHWAR) design a clear, efficient system for collecting and sharing data — giving voice to the real impact of their training program and elevating the role of CHWs in community health.
